When you need CAPR to verify your credentialling or exam status, you can now submit a Verification Request Form directly through the client portal. This form enables us to confirm your status to the regulator in the province or territory where you are applying, or to another agency of your choice.
How to submit a Verification Request Form online
- Log in to the Client Portal: Go to the Portal Login and log in with your username and password.
- Apply: In the Apply tab of your profile, select ‘Create Verification Request Form’.
- Tell us who to send it to: Select the relevant regulator from the drop-down menu. Or, select ‘Other’ from the drop-down menu, then enter the email address of the relevant agency or person you want the completed form sent to.
- Submit: Review your information and click ‘Submit’.
Timeframe: CAPR will send your completed Verification Request Form within 5 business days.
